After scheduling your initial appointment, you can preregister by downloading, printing, and completing the forms provided below. Be sure to bring your completed forms, any test results, and diagnostic imaging films with you to your first visit. If you have questions about insurance coverage, please don’t hesitate to contact our office, as we are an out-of-network provider.
During your first visit, you will be welcomed by our friendly staff and introduced to Dr. Arien J. Smith, MD, FACS, who will take the time to understand your needs. Dr. Smith will review your medical history, discuss any current symptoms, perform a thorough examination, and evaluate any diagnostic tests you’ve previously undergone. To ensure a complete assessment, please bring your completed new patient forms, photo ID, insurance card, any physician referrals, and relevant medical records.
At the conclusion of your appointment, Dr. Smith will develop a personalized treatment plan tailored to your condition and goals.
